COL Littlefield's name carries a handwritten initialling mark struck through it, consistent with the routing having been executed. The file reference 'SAVE-849-78' is handwritten at the foot, and a handwritten 'CY 1 OF 3 CYS' appears at lower right.

The typed signature block is partly overprinted by a declassification stamp reading 'CLASSIFIED BY ... EXEMPT FROM GENERAL DECLASSIFICATION ...' with the handwritten annotation 'Cdr, USA INSCOM'. The words 'LTC' and 'Commanding' are legible under the overprint but the line spacing is disturbed; the rank line reads LTC and the title line reads Commanding. Keenan is named in paragraph 1 as one of the two officers who attended the meeting, so the memorandum's author is also a participant in the event it records.
